DIETARY PROGRAM
At RiverPointe Post-Acute, we understand that nutrition is essential to our residents’ wellbeing. We serve specialized diets to meet each resident’s needs. We are pleased to offer a wide variety of food to meet each individual’s preferences. Whether it is softer foods for a resident who is unable to chew well, or a low sodium diet-- we can meet the challenge.
Upon admission, our well-trained Dietary Services Manager will meet with you to discuss your preferences. A registered dietician and your doctor will finalize your personalized diet. This will ensure you can get what you need to assist in your recovery.
Examples of our dietary services:
Provide meals to meet specific dietary and therapeutic needs, according to physician orders. (Please inform the dietary supervisor in advance regarding all substitute menu requests)
Oral supplemental diets are available
Regular nutritional assessments from our registered dietitian

Each patient is provided a monthly calendar with upcoming events such as birthday parties, movies, musical programs, outings, ice cream socials, and other special events. We always welcome friends and families to visit and attend our special events!

Physical Therapy
Physical therapy focuses on identification and rehabilitation of individuals with physical impairment and pain, as well as future injury prevention. Following evaluation, the physical therapist develops an individualized treatment program.
Areas of treatment focus may include:
Increasing strength and mobility in the body and lower extremities
Increasing motor control for a patient who has suffered a stroke
Improving coordination, balance, and the ability to walk following orthopedic surgery
Improving overall strength conditioning, range of motion and endurance following hip or knee replacements
Teaching pain management
Teaching proper body mechanics to prevent injury
Promoting safety awareness for the well-being of the resident
Orthopedic Unit
RiverPointe Post-Acute is pleased to introduce our new Orthopedic Unit. We listened to the needs of our community and the requests of the local hospitals to expand our rehabilitation services. We have dedicated a wing of our facility to handle the sophisticated needs of patients after total hip, knee, or shoulder replacement and/or repairs to injured joints. This area of the facility serves the special needs associated with these types of procedures. All rooms are two-bed rooms with patients only coming in for short-term rehabilitation services.
Our rehabilitation team assists patients and their families during this transitional period from surgery until they are discharged back home or to a lower level of care. We provide essential information to help make a successful transition. We are open to your concerns and feedback. We will work with our discharge planner to make all the arrangements when you have completed your first phase of recovery. For questions, please feel free to ask your therapist or our Director of Rehabilitation. In our unwavering commitment to continuous improvement and the provision of world-class patient care, we welcome and appreciate your feedback to assist us with addressing any of your questions or concerns.
Our experienced rehabilitation team is committed to the quality of care for our residents. Patients are empowered through individualized programs to reach goals of increased independence. Patient families are also encouraged to participate in therapy sessions.

Speech Therapy
Our speech therapists provide a myriad of services including assessment, education, and treatment for language, voice, articulation, and fluency disorders. Our treatment programs are directed towards improving communication skills and safe swallowing. We assist in rehabilitating patients who struggle with comprehension, memory, processing, and expression abilities. We also instruct the patient or the caregiver in the techniques ranging from head positioning to promoting safe swallowing at a slower rate to clearer speech. We also teach patients how to compensate if they cannot be rehabilitated to their prior level of performance.
Speech therapists work to restore the following abilities:

For the individual needing long-term skilled nursing care, the attending physician and facility staff will do what they can to assist the individual with the activities of daily living and mobility based on each individual’s circumstance. It is important to understand that advancing age brings with it the inevitable effects of aging that no medical care or treatment can change or cure.
As the aging process advances, our nursing services will shift to end-of-life care. The staff at our facility will assist in making you and your loved ones as comfortable as possible as the end of life nears. Unfortunately, we cannot predict exactly when end-of-life care will begin; it very much depends on each individual’s ever declining health condition which is a part of every human being’s life. It is important for both you and your loved one to discuss with your physician and facility caregivers the natural history of the aging process.
Your attending physician and our facility staff will help you understand the risks and benefits of care choices being made with you and your loved one. We ask you to understand that in spite of compassionate care, attention, and treatment, chronic medical conditions may continue to progress. The inevitable effects of aging and the progression of chronic illnesses are the realities of life, but we're here to provide quality long-term care with compassion and dedication as the aging process continues.
